Improving referral management & patient care with breakthrough technologies for home health: An executive provider discussion

When the healthcare industry first faced the onset of the COVID-19 pandemic, home health providers experienced an initial drop in referrals. As nationwide efforts to control the spread of the virus developed, demand for care in the home accelerated, leading to a rapid increase in referrals to home health throughout the rest of 2020 and into 2021. To keep up with increased referral volume and manage the patient journey, home health providers are leveraging digital tools to manage referral relationships, coordinate care, and track analytics.

Watch a panel discussion sharing successful provider strategies for refining referral management and tracking patient care with CarePort®, powered by WellSky. This panel includes Mike Gregory, RN, Chief Patient Advocacy Officer at Intrepid USA Healthcare, Inc. and Susan Caputo, Vice President of Business Development at VNSNY.

Moderated by Kraig McKinley, Area Vice President, Post-Acute for CarePort, the discussion shares insights on:

  • Shifting referral patterns and engaging with referral sources
  • Starting patient care at the initial referral
  • Leveraging information throughout the patient journey to manage to wellness or through disease progression
  • Evaluating patient populations for outcomes

Intrepid and VNSNY have used CarePort to effectively manage increased referrals over the last year, giving both organizations real-world insights on leveraging digital tools for growth and better care. Don’t miss this opportunity to learn from successful home health leaders!
