Annette is a registered nurse with a master's in health care administration and has been practicing since 1990, with most of her nursing experience in the home health and hospice arenas. For over a decade, she worked with the CMS Medicare Administrative Contractor (MAC) where she provided reviews and education of home health and hospice documentation. Annette then began providing education regarding Medicare reimbursement issues and effective documentation strategies, assisting providers with ADRs and appeals. Today she marries together her experience in the “real world” and her inside knowledge of Medicare to ensure providers can meet the requirements of CMS and run compliant, efficient operations. She presents and consults nationally on the Conditions of Participation, (COPs), OASIS, PDGM, HIS, LCDs, and documentation and coverage for both the home health and hospice benefits. Annette holds both the Certificate for OASIS Specialist-Clinical and the Home Care Coding Specialist-Diagnosis certification.
Thomas (Tom) Martin, Director of Post-acute Analytics at CarePort, powered by WellSky, has led several data analytics teams, providing insight to healthcare providers trying to improve quality of care for their post-acute care patients. He holds a master's degree in resources economics with a concentration in econometrics from the University of Massachusetts Amherst. Tom studies how the ever-changing post-acute care regulatory and payment landscape is impacting care delivery and how acute and post-acute providers can leverage their data to improve patient care.

Sara has over 20 years of experience building cutting-edge, technology-based solutions. For the last 5 years, she has led our Guide, Connect and Insight, and Ambulatory and next generation solution suite for WellSky. She currently leads the WellSky CarePort Post-Acute Care (PAC) solution suite where she is designing, innovating, and developing smarter tools for PAC providers to better meet the needs of the industry. She is passionate about improving our healthcare system with a focus on improving patient outcomes while also reducing spend.
