Strategies to improve assessment accuracy

In January, we hosted a webinar, Home Health Value-Based Purchasing: Lessons learned and strategies to improve patient outcomes. Due to popular demand, we now present a deeper dive into the discharge (DC) function score as well as strategies that home health agencies can use to improve assessment accuracy for the OASIS, which is critical to an agency’s Total Performance Scores (TPS).

While the Section GG data elements have been part of the OASIS data set for some time, they have often been underprioritized and inaccurately reported due to their limited direct impact on publicly reported outcomes and reimbursement in home health. As an experienced physical therapist and a subject matter expert in OASIS, she has a strong understanding of how to efficiently assess and accurately translate patient status and functional ability in the home to the OASIS, which affects all aspects of agency health.
