Bridgette McGrath

Bridgette McGrath is a results-driven executive with a strong background in healthcare management. With experience overseeing the $600 million profit and loss account and day-to-day operations at CareBridge, as well as executing multi-site management at Healogics, Bridgette has a proven track record of driving efficiency and achieving excellence in patient outcomes. Her ability to unite stakeholders, introduce innovative strategies, and optimize processes has consistently delivered exceptional results in the healthcare industry. 

Jaclyn Rowbotham MSN, RN, ACM-RN

Manager, Quality & Education/Complex Care/Care Progression Mgrs Integrated Care Coordination at Lehigh Valley Health Network Part of Jefferson Health

Jaclyn Rowbotham is an experienced healthcare leader with over 18 years in nursing, quality improvement and care management. She currently serves as the Manager of Quality and Education for Care Management across the Jefferson Health System, where she leads education strategy, staff development and performance improvement initiatives across integrated teams.
Jaclyn’s past roles encompass Ambulatory Specialty Care and Director of Quality and Clinical Risk management where she oversaw multi-site operations, quality programs, infection control and patient safety efforts. With deep expertise in PDCA methodology, regulatory compliance and interdisciplinary collaboration, she has driven measurable improvements in patient outcomes and care coordination.
Jaclyn currently holds a master’s in nursing administration and has her certificate in Accredited Case Management. Jaclyn is a published author on quality and safety topics and has presented a various local and national conferences throughout her career.

Karine Shnorhokian, MSN, RN, RAC-CT

Vice President of Case Management & Value-Based Care at Care One Management LLC

Karine Shnorhokian has been with CareOne since 2019, where she now serves as Vice President of Case Management & Value-Based Care, driving strategy, engagement, and process improvement to enhance patient outcomes. She began her career in adult critical care and later advanced to leading case management and clinical integration, building effective discharge planning models. Before joining CareOne, she directed value-based care initiatives, managing an ACO of more than 10,000 lives, and pioneered simulation-based education for care transitions.
Karine has presented nationally at ACMA, AAPACN, NAACOS, NICHE, and the Society for Simulation in Healthcare, highlighting her work on inpatient mobility and complex care transitions. She has been featured in The Wall Street Journal and on Fox 5 News for launching an innovative harmonica program for patients with chronic lung disease.
With over 20 years of healthcare experience, Karine holds a Master of Science in Nursing with a leadership concentration and currently serves as President of the NJ ACMA chapter. Beyond her professional roles, she is active in the Armenian community and testified before Congress in 2015 on genocide and human rights.

Alicia Tennenbaum, LCSW

VP of Care Management at Med-Metrix

Alicia Tennenbaum, LCSW, is a licensed clinical social worker in the state of New York with over two decades of leadership in healthcare. She earned her B.A. from Villanova University and received her Master of Social Work from New York University.
Since 2001, she held progressive leadership roles at the Mount Sinai Health System, including Associate Director of the Department of Social Work and Case Management for the Mount Sinai Beth Israel, Mount Sinai Union Square, and Mount Sinai New York Eye and Ear campuses and as Executive Director of Social Work and Care Transitions for the Mount Sinai Health System.
She is currently the VP of Care Management at Med-Metrix, a leading Revenue Cycle Management company helping healthcare providers enhance patient experience and optimize revenue.
Alicia is an active board member and Secretary of the New York City chapter of the American Case Management Association (ACMA). She was featured in the New York Times Science section on June 19, 2007, in the article "In the Trenches, the Hospital Discharge Planner: Bridging Gaps Between Hospital and Home."
Her academic contributions include co-authoring the chapters “Preparing for Your Discharge” in Questions Patients Need to Ask and “The Practice of Clinical Social Work in Healthcare”.
She has a certification in Bioethics and is a graduate of the Cornell-HANYS Academy for Healthcare Leadership Advancement. She has been recognized with several honors, including the Mount Sinai Heart Award and the Ida M. Cannon Award from the Society for Social Work Leaders in Health Care.
