Webinar Caring Smarter

People who choose a life of caring — and the organizations and communities they serve — deserve a special kind of support.

Lessons learned from applying care transition technology to improve outcomes across the acute and post-acute continuum

Originally presented at the Home Health Care News Capital & Strategy Conference on
December 9, 2020.

A panel discussion presented by:
  • Samir D'Sa, General Manager, Landmark Health
  • Dr. Lissy Hu, CEO, CarePort
  • Bill Miller, CEO, WellSky
  • Shoma Thomas, CMO, WellSky
As providers across the care continuum take on more risk, effective collaboration is vital for powering better outcomes for patients, payers, and providers. WellSky participated in a panel discussion on this topic during the Home Health Care News Capital & Strategy Conference. In this informative discussion, four healthcare industry thought leaders address the practical application and long-term potential of care coordination technology, as healthcare continues to shift into the home and towards risk-based agreements.

WellSky CEO Bill Miller, CarePort CEO Lissy Hu, MD, and Landmark Health General Manager Samir D’Sa share their perspectives on the importance of harnessing the power of coordinated care – particularly in the context of value-based care models. Landmark Health is a risk-based provider group that delivers high quality, primary and urgent medical care to chronically ill Medicare Advantage patients, right in their homes.

The speakers discuss lessons learned through implementing care coordination programs in home-based care settings; how technology can streamline care transitions and referral management; and their vision for the future of truly interconnected care.
