Partnerships between CoCs & health care entities:

How they enhance coordinated care, leading to sustainable, successful outcomes for the homeless population

Continuums of Care (CoCs) understand the challenges involved in building and leading a support system for the homeless population. They have been capturing and aggregating data for years to help gain a broad perspective on the challenges facing those experiencing homelessness, with the objective of producing improved, sustainable outcomes.  

The next step in the evolution of coordinated care is to develop formal partnerships with health care systems and professionals so health outcomes can be improved at a faster pace. CoCs and health system providers must have a cooperative and communicative relationship to truly convert homelessness from a short-term challenge into a solution that not only provides housing, but ultimately helps people resume healthier, more independent lives.

Gabriel (Gabe) Cate is responsible for strategic growth initiatives across WellSky's Human and Social Services group, focused on County and Local government, coordinating care coalitions, and community-based organizations. Previously, Gabe served as the Chief Operating Officer at Bowman Systems, LLC, which developed the first commercially available HMIS system in the industry and was acquired by WellSky in 2016. Gabe has presented at conferences on topics ranging from SDoH to technical solutions for data integration challenges.

Ronald W. Lanton III, Esq., has over 25 years of experience focused on government affairs including activities on the municipal, state, and federal government levels with 15 years dedicated to the healthcare sector. Ron founded the healthcare government affairs and lobbying firm True North Political Solutions. He is now the Executive Director and head lobbyist at Frier Levitt Government Affairs, LLC and is also Senior Counsel at Frier Levitt.

Ron has also worked at Stateside Associates, a greater Washington D.C. area government affairs consulting firm where he handled healthcare, energy, commerce and transportation issues. He has also clerked for a federal magistrate and was appointed as a municipal commissioner on beautification and environmental issues. He frequently consults Wall Street firms on financial issues related to the healthcare sector.

Ron is a 40 under 40 award recipient from the Springfield, Illinois Business Journal. He has been admitted to practice in New York, Illinois and the District of Columbia. He has a BA from Miami University and a JD from The Ohio State University. He is also the chair of the Biologics Committee for the New York Bar Association.
