Social determinants of health (SDoH) have become a driving force for public health and community action. This webinar will focus on the health and wellness factors of SDoH, and their impact on value-based care (VBC), a key part of healthcare reform.

You will also gain actionable ideas on how government agencies, non-profits, and healthcare providers can work together on SDoH to help their communities thrive. Topics include:

  • How VBC payment models are changing healthcare
  • The definition of SDOH and its relevance to VBC initiatives
  • Health System - CBO partnerships to reduce healthcare costs
CoCs (Continuums of Care), area agencies on aging, and other community-based organizations (CBOs) are well-positioned to partner with health systems because of their existing service networks and deep ties to the community. The infrastructure these agencies already have in place also appeals to health systems and payers. In return, additional resources from the healthcare industry can help to advance the goals and missions of CBOs.
