About the seminars:

Building the bridge to hospice: Leveraging connections between home health, palliative, and hospice care

CE credit offered – approved for up to 1.00 ANCC contact hours

To best manage and support a patient life over time, providers of care in the home must understand and embrace the navigation into end-of-life care. All too often, gaps occur in transitions from home health to palliative to hospice care, particularly when it comes to moving patients at the right time. Providers can bridge these gaps to support a connected care ecosystem, improve care quality and outcomes, lower costs, and bolster agency growth.

Join us for a free virtual seminar that will help you embrace your organization’s position within this ecosystem and build stronger connections between home health, palliative, and hospice care. In this seminar, two expert consultants and a successful provider will offer clinical and operational strategies for improving care transitions. The presenters will share actionable guidance to enhance the effectiveness of these transitions to improve both the patient experience and the overall success of your organization.

Attend to learn:

  • The most important aspects of service transition between home health, palliative, and hospice care
  • Commonly found gaps in the progression of care planning and how to mitigate them
  • Practical solutions for achieving smoother, more timely patient transitions to effective end-of-life care

Presented by Cindy Campbell, MHA-Healthcare Informatics, BSN, RN, COQS, Director of Operational Consulting for Home Health, WellSky; Jody Holmes-Dion, Director of Operational Consulting for Hospice, WellSky; and Bob Parker, DNP, RN, CENP, CHPN, CHP, Chief Clinical & Compliance Officer, Intrepid USA

Turn up the volume! How to increase referrals in a value-driven market

The events of 2020 turned the home health industry upside-down, requiring providers to reevaluate and rapidly adapt to new and continuously changing market conditions. In an environment geared toward value-based initiatives, home health is a key partner. Many factors are at play in amplifying – or hindering – overall success: how agency leaders guide the focus of clinical and operational teams, how sales and marketing teams work, how partners perceive the organization within the market, and how those partnerships are built. How will you take advantage of the current and future state of the industry?

Join us for a free, virtual seminar that will help you understand, enhance, and position your organization’s value, leveraging your presence in the market to grow and succeed. In this seminar, two respected home health consultants will share strategies to rise above the noise of the competition and turn up the volume on your referrals.

Attend to learn:

  • Why changes in the home health payer market mean growth opportunities for you
  • How to recognize and overcome common barriers to strategic growth
  • Key sales strategies for positioning your agency with payers and referral sources

Presented by Cindy Campbell, MHA-Healthcare Informatics, BSN, RN, COQS, Director of Operational Consulting, WellSky; and Melanie Stover, OT, MBA, MS/ISM, Partner, Home Care Sales
