Quality of care suffers when documentation is absent or lacking. Knowing social drivers of health have a significant impact on health outcomes, multi-sector stakeholders from around the country are working together to develop new data standards around SDoH, which could change the future of healthcare.
In this panel, we will discuss the implications of standardizing SDoH in the clinical setting. From leveraging interoperable technology to expanding the ICD-10-CM Z codes, learn why data collection, sharing, and reporting is going to prove critical to building, implementing, and scaling successful SDoH interventions.

Katiya has spent the last decade exploring how health care intersects with technology, specifically coordinating efforts to use technology to further patient-centered and whole-person care. She has provided project leadership and standards development support for care coordination projects on behalf of various federal agencies like National Coordination for Health Technology (ONC), Center for Medicare and Medicaid Services (CMS), and the Center for Disease Control and Prevention (CDC).

John oversees strategies that transform the way communities address social determinants of health, through digital innovation, provider alignment and deployment of its proven model for unified intake, referral, engagement and navigation. Under John’s leadership, Care Connect + has built a network of over 110 community-based organizations across Northeast Florida, advancing key health factors like homelessness, mental health, social isolation and overcoming systemic barriers, to positively affect change and build healthier communities.
Linda Hyde, RHA
Lead Terminologist for The Gravity Project
Linda is a consultant on projects related to electronic data collection for performance measurement and other initiatives. She regularly works with various classification and terminology systems including ICD-9, ICD-10, RxNorm, LOINC, and SNOMED CT®. Ms. Hyde has served as chair and member of the AHIMA Classification and Terminology and the Quality Measurement and Secondary Data Use practice councils and was a member of the NQF Expert Panel on Coding Maintenance. She is currently a member of the AHIMA Advocacy Practice Council.
Manik Bhat
Vice President of SDoH Operations and Sales at WellSky
Manik co-founded Healthify in 2013 while working as a community health worker in East Baltimore. He saw how social needs - like access to food and housing - dramatically impacted health outcomes in the community and was moved to create Healthify. Manik is a thought leader in the social determinants space, an avid speaker, and an advocate for addressing the social determinants of health through equitable and collaborative partnerships between healthcare and social service organizations. Healthify was acquired by WellSky in 2021.
