The Southern California Care Coordination Summit connects leaders and executives from hospitals, health systems, and physician groups for an event featuring educational sessions exploring market trends, care coordination best practices, and how technology is breaking down silos across the continuum.

1:30 - 2:15 PM: Leveraging real-time alerts to improve patient transitions across the continuum

  • Tess Thaler, MHA, Manager, Post-Acute Network, Scripps Health
  • This session highlights how Scripps Health uses the WellSky CarePort Connect solution to proactively manage transitions of care for ACO patients. Attendees will learn how real-time alerts notify case managers when patients are discharged from hospitals into an in-network skilled nursing facility (SNF), enabling timely assignment to clinical teams and ensuring continuity of care. The session also explores how alerts on SNF discharges trigger follow-up coordination with primary care providers, reducing the risk of readmission. Participants will gain insight into how digital tools can be leveraged to close care gaps, enhance patient tracking, and advance value-based care strategies.

2:30 - 3:30 PM: Healing without a return ticket: Rethinking the discharge journey

  • Marie Stinebuck, MBA, MSN, RN, ACM, Chief Operating Officer, Phoenix Medical Management Inc.
  • This session explores the evolution of value-based care and its continued impact on improving patient outcomes and reducing readmissions. Attendees will gain insight into recent CMS proposals to include Medicare Advantage plans in readmission measures and learn about the upcoming Transforming Episode Accountability Model (TEAM), which introduces new financial and quality expectations for hospitals. The presentation will highlight the pivotal role of case management leadership in navigating these changes through effective care coordination, patient engagement, and outcome-driven strategies.

Meet our speakers!


Marie Stinebuck, MBA, MSN, RN, ACM, Chief Operating Officer, Phoenix Medical Management Inc.

Marie Stinebuck is the Chief Operating Officer of Phoenix Medical Management. With nearly 30 years of nursing experience, she has spent almost 20 years specializing in case management. Her career has been primarily focused on the acute hospital setting, including the implementation of outpatient case management programs. Marie has held executive leadership roles overseeing case management, utilization review, denial prevention, clinical documentation improvement (CDI), and medical record integrity. 

Marie is also the author of numerous articles related to case management, utilization review, and denial prevention. Her true passion is education and she has been instrumental in expanding Phoenix Medical Management's education program over the past 5 years. Marie has served on the Arizona Board of the American Case Management Association (ACMA) and is certified through the ACMA. She holds an MBA and a Master of Science in Nursing (MSN) with a focus on Nursing Leadership.


Tess Thaler, MHA, Manager, Post-Acute Network, Scripps Health

Tess Thaler is the Manager of Post-Acute for Scripps Health. In her role, Tess has developed value-based skilled nursing facility and home health networks to support the care of Scripps patients throughout the care continuum. Through relationships with post-acute partners, she oversees continuous improvement processes with the goal of providing higher quality care and patient experience in the post-acute setting, while managing costs for the organization and value-based populations. Prior to her current role, Tess was the project manager for the Scripps ACO.

Tess earned her Masters in Healthcare Administration from USC and a Bachelor of Science in Human Biology from UCSD. During her free time, she enjoys reading a good book and the San Diego sunshine.

April Vindiola, LCSW,  ACM-SW, Director of Case Management, PIH Health Good Samaritan Hospital

April Vindiola is a seasoned healthcare leader and licensed clinical social worker with over a decade of progressive experience in hospital-based case management. Currently serving as Director of Case Management at PIH Health Good Samaritan Hospital in Southern California, April oversees operations across this campus, where she leads multidisciplinary teams in driving quality outcomes, care coordination, utilization review, and strategic discharge planning. April is known for her innovative approach to care management, having implemented system-wide protocols that have improved compliance, efficiency, and patient transitions—especially in high-risk and complex cases. A dedicated advocate for vulnerable populations, she has developed partnerships with community agencies to strengthen post-acute care solutions and reduce hospital readmissions. 

As an executive master's candidate in Healthcare Administration at Cornell University, graduating in May 2025, April brings a cutting-edge understanding of healthcare systems, policy, and leadership. Her academic background also includes a Maser of Social Work with a concentration in Health from the University of Southern California. Trilingual and bicultural, April combines cultural competence with clinical expertise to lead with empathy, clarity, and impact. She is also the active Treasurer for the American Case Management Association's Southern California Chapter. April's voice in healthcare speaks to the intersection of compassionate care, operational excellence, and strategic leadership. She brings these insights to the stage as a respected speaker on case management innovation and care delivery transformation.
