In 2017, the leadership of Flagler Health+ found themselves in a perfect storm. Sixty percent of their emergency department visits were completely inappropriate, due largely to inadequate housing, food insecurity, and other unmet social needs. Readmissions to the hospital were also unnecessarily high. And the teen suicide rate in their prosperous Florida county was 3.2 times the national average.
Today the situation is changing rapidly. Flagler Health+ launched Care Connect+, a network of 65 local agencies and healthcare professionals to form one community alliance that addresses social determinants of health. Every organization in the alliance — from homeless shelters to food pantries, to clinics — share a single system for intake, assessment, eligibility screening, referrals, and care coordination.
