How CBOs can address social determinants of health

How CBOs can address social determinants of health

Mounting evidence suggests that social determinants of health (SDOH) — such as unstable housing, food insecurity, and interpersonal violence — have a tremendous impact on health outcomes and health costs.

Health care providers and payers are now looking for ways to address SDOH and enhance their efficiency. This creates new opportunities for community-based organizations not only to participate, but to take the lead in addressing the key factors affecting the health of their communities.

This session helps human services organizations work more effectively with the health care system and other community stakeholders to:

  • Craft partnerships that align with your mission
  • Bring high quality services to your community
  • Create sustainable funding solutions for your organization

You will gain actionable advice to meet some of the greatest needs in your community. This webinar is designed for any agency that coordinates care, including community-based organizations, HUD continuums of care, area agencies on aging, and state agencies that coordinate human services.
