Rainna Bernesser is the force behind the innovative Allegheny County Care Transitions program. As a leader in quality performance, she has built and continues to refine an effective and forward-thinking process improvement for the Care Transitions Program. Rainna has 19 years’ experience working with the Area Agency on Aging, initially as a program analyst, then a project manager, and now the director of special services and strategic planning bureau at the Allegheny County Area Agency on Aging. Rainna holds two master’s degrees: one in social administration and one in public non-profit management from the University of Pittsburgh.
Darren Cerrato has worked for 17 years across multiple human service domains managing custom, state, and/or federal software solutions. Currently, he is a Senior Analyst in DHS’ Analytics, Technology, and Planning Office. As part of Darren’s current role, he manages the Area Agency on Aging’s software solutions, as well as ensuring all data sources are integrated with our DHS Datawarehouse, which contains 124 different data streams from various internal and external sources across Allegheny County.
