Reducing hospital readmissions with home-delivered meals: Results from a hospital-community collaboration

According to the American Society of Parenteral and Enteral Nutrition, 30-day hospital readmission rates are 160% higher in patients who are malnourished. Food insecurity is a major public health problem in the United States, and it contributes substantially to the burden of chronic disease.

In an effort to identify ways to reduce hospital readmissions due to lack of access to healthy food when returning home from a hospital stay, Meals on Wheels America conducted a 10-month study* to determine the impact of a meal intervention program. This study included two local Meals on Wheels America programs in Florida and New York collaborating with area health providers to receive client referrals once they were discharged from a hospital stay. Through this study, Meals on Wheels America saw a decrease in hospital readmission rates and improved health outcomes -- further demonstrating the importance of partnerships between healthcare and community care organizations.

For over a decade, she worked with the CMS Medicare Administrative Contractor (MAC) where she provided reviews and education of home health and hospice documentation. Annette then began providing education regarding Medicare reimbursement issues and effective documentation strategies, assisting providers with ADRs and appeals. Today she marries together her experience in the “real world” and her inside knowledge of Medicare to ensure providers can meet the requirements of CMS and run compliant, efficient operations. She presents and consults nationally on the Conditions of Participation, (COPs), OASIS, PDGM, HIS, LCDs, and documentation and coverage for both the home health and hospice benefits. Annette holds both the Certificate for OASIS Specialist-Clinical and the Home Care Coding Specialist-Diagnosis certification.
