People who choose a life of caring — and the organizations and communities they serve — deserve a special kind of support.
When Reading Hospital conducted a gap analysis of Berks County, PA (the area served by the hospital), that analysis showed a food-insecure population that included low-income people, households with children, immigrants, minorities, people with disabilities, and those living in food desert areas. Nearly 69% of respondents stated that in the past 12 months, they often worried that their food would run out before they could buy more. In response, the hospital brought together a consortium of clinical and community-based organizations (CBOs) with a specific aim: to enhance food access for vulnerable populations in Berks County.
This case study details how Reading Hospital was able to mobilize the community to improve food access, as well as the health outcomes that resulted from addressing this key social determinant of health (SDoH). An analysis of a study cohort with resolved food needs showed that:
The results demonstrate the value of collaborative work between hospitals and CBOs. These stakeholders relied on a closed-loop referral technology platform that equipped healthcare providers to more easily refer a patient experiencing food insecurity to the help they needed. The platform ensured more referrals were fulfilled, enabled greater collaboration among community providers, and delivered more detailed outcomes data that led to better decision making.
