No home health agency wants deficiencies after a survey – especially not condition-level deficiencies. Condition-level deficiencies are serious and indicate that an agency is not in compliance with the Home Health Conditions of Participation (CoPs). The top condition-level deficiency in 2018 was §484.60 Care planning, coordination of services, and quality of care.
