In 2016, the Centers for Medicare & Medicaid Services (CMS) introduced a service intensity add-on (SIA) payment for routine home care visits by a registered nurse or social worker to meet the increased clinical and emotional needs of patients in the last 7 days of life and their families. However, after two years with this additional benefit, skilled visits at the very end of life have not significantly increased. In her role at Fazzi, Catherine serves providers across the country, focusing on educational development, optimizing organizational design, operational efficiency, advanced clinical modeling and quality improvement programs, and assisting organizations in achieving best practice clinical and financial performance. As a hospice and palliative care certified nurse and certified hospice case manager, Catherine reaches into the hospice philosophy by integrating a strong set of patient-centered core values and smart, sustainable business models.
Jill Dyer, BSN, RN, HCS-D, HCS-O is a senior-level home health executive with more than 30 years of progressive management experience. An accomplished nurse, Jill has fulfilled a wide variety of roles that inform her expertise as an industry consultant, including home health administrator, home health director of nursing, and nurse consultant for home health. Jill is also certified as a Home Care Coding and OASIS Specialist. Through her clinical knowledge as a nurse and extensive management experience, Jill brings a comprehensive and well-rounded perspective to her work as a consultant and educator.
