Webinar Caring Smarter

People who choose a life of caring — and the organizations and communities they serve — deserve a special kind of support.

Three ways payers can tackle challenges related to the social determinants of health

Studies show that up to 81% of patients withhold information from their providers. This makes addressing social determinants of health (SDoH) in a cohesive, systematic way difficult. When this information isn’t recorded, payers cannot partner with providers to create timely and necessary interventions that reduce the impact of various social risk factors.

In this resource, we’ll explain the importance of SDoH initiatives and why they often fail. More importantly, we’ll suggest three ways you can leverage member data to build targeted interventions that can help improve community health outcomes across vulnerable populations.

Download the tip sheet to learn how:

  • To gain a better understanding of how social barriers are impacting members in your network
  • Data can be used to partner with providers to create interventions that lower costs and improve outcomes
  • Supplemental benefits can help specific populations reduce the impacts of social risk factors
