Your documentation is your testimony: Understanding the importance of documentation in rehabilitation

During the COVID-19 public health emergency, many things have changed including the pausing of surveys and audits in many post-acute settings of care. Recently, the Centers for Medicare and Medicaid Services (CMS) and other federal agencies have authorized inpatient rehabilitation audits to resume. As these audits resume, our teams must reiterate the importance of documentation.

Providers may be able to appeal denials from CMS contractors and other federal agencies. However, providers do not have the opportunity to verbally defend the delivery of care until the third level of the appeal process. Instead, from the initial review of the record through the second level of the appeal process, the documentation is the only “testimony” of the delivery of care considered by CMS.
